The Division of Infectious Diseases is seeking to fill a Department Coordinator III position which will be responsible for coordinating the establishment and continued monitoring of service outcomes related to the Ryan White Part C/D Program Services. This position works in conjunction with the Ryan White team and community partners to assist in the development of processes to identify and locate new clients and those who have fallen out of care, in order to bring or re-engage individuals into care and treatment. To accomplish this goal, the Department Coordinator III will review and analyze program data to identify and establish outreach and engagement priorities; coordinate the development, promotion, and implementation of a variety of program initiatives designed to improve access to care and patient retention; and build and maintain relationships with key community partners for improving patient care and health outcomes. The Department Coordinator III will also create and evaluate client needs assessments, and report on demographic performance measures and loss/retention rates for the program.
Essential Duties and Responsibilities
Coordinate the development, promotion, and implementation of a variety of program initiatives designed to improve access to care, linkage to care, and patient retention in care, including support groups, health fair, AIDS walk, Pride, etc.
Work with Ryan White Program staff to review, analyze program data to identify and establish community outreach and engagement priorities to improve health outcomes.
Develop new and maintain existing relationships with community partners for improving patient care and outcomes.
Create and evaluate client needs assessment, demographic performance measures for target populations, and generate monthly reports on loss/retention rates.
Collaborate with medical case management team with goal of establishing target areas of greatest care concern.
Update and monitor Ryan White Program social media presence.
Other duties as assigned.
This is an in person position, working 5 days a week.
Target Compensation Maximum:
$27.85Target Compensation Minimum:
$18.56Compensation will be commensurate to candidate experience.
